Creative Therapy Event Pre-Registration Form

This form is provided for pre-registration purposes so that we will hold a place for the desired class or camp.  We will be in communication with you for formal registration along with payment prior to the event.  This gives us a chance to answer any questions you may have about the suitability of an event for your particular circumstances.  There is no obligation on your part at this initial stage.  If you have pre-registered for an event but subsequently decided that you will not be able to participate you can simply call the office at 519-954-5026 or send an email to info@creativeot.com to let us know.
